QUIT PLAN MANAGER Automates Tobacco Treatment for Cancer Centers. It gathers patient data from the EHR of all tobacco users on their first visit; assigns each to the most appropriate tobacco care pathway; eliminates the need for manual referrals; follows-up utilizing personalized automated voice calls, emails, and text messages to ask patients about their current smoking status and medication usage.
At each contact, patients are offered a referral to helpful resources such as the state Quitline. Timely triage results in more successful quits.
Returns outcomes to patients’ charts in the EHR.
